Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish to each of his employees employment and a place of employment which are free from recognized crush-by hazards that are causing or likely to cause death or serious physical harm to his employees: (a) On June 6, 2017 the employer did not ensure adequate clearance was maintained from overhead obstructions while employees adjusted cable from an elevated aerial lift, exposing employees to being caught-between the railing of a boom lift and a red-iron beam.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) On June 6, 2017 employees were exposed to head injuries from impacting a red-iron beam while adjusting cable from an elevated boom lift without wearing protective helmets.
