29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Divider Area, on or about 5/1/17: Point of operation guarding was not provided to prevent the employees from reaching inside of the Divider Machine hoppers to remove dough. On 5/1/17 a machine operator reached into the hopper of Divider #1 and sustained a right index finger tendon laceration when the knife came down on his hand. b) Divider Area, on or about 5/1/17: Point of operation guarding was not provided to prevent the employees from reaching inside of the Divider Machine hoppers to remove dough. On 5/1/17 a machine operator reached into the hopper of Divider #2 and removed a ball of dough. The operator did not turn the machine off and was thereby exposed to the point of operation.
