29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about June 6, 2017, at the worksite located at 2407 Phillips Drive in Jonesboro, Arkansas, employee in an elevated man basket to a JLG 860SJ extensible boom aerial lift, did not wear fall protection while installing tile to the exterior of the building. Employee was exposed to tip-over and falls from elevation.
