29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    On or about June 7, 2017, and at times prior thereto, in the Manhole Department and Pipe Department floor openings approximately 4.5 feet high was not properly guarded by standard railings.  Employees were exposed to  fall hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about June 7, 2017, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:     1) In the cutting/bending department, an iron worker machine was found not having guards provided.     2) In the cutting/bending department, a rebar cutting machine operated via foot pedal was not provided with a guard to cover the blade.     3) At the wet cast area, a roller being operated was not provided with tripwires or bars to shut the roller off in case of emergency.     Employees were exposed to possible amputation and crushing hazards.

Employees were exposed to  amputation hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    On or about June 7, 2017, and at times prior thereto, at the mixer area in Manhole Department a 240-Volt electrical panel with live electrical parts exposed was not guarded against accidental contact.  The employees were exposed to electric shock.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about June 7, 2017, and at times prior thereto, at the establishment the employer did not establish a Lockout/Tag-out program consisting of an energy control procedure, employee training and periodic inspections in maintenance testing operations.  Employees were exposed to unexpected release of energy hazards.
