29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  (a) New residential construction located on Lot #503, Vacant lot across the street: On or about 06/08/2017, and at times prior, the employer exposed employees to amputation hazards while using a Stow Mortar Mixer with exposed unguarded moving parts, in that the starter recoil on the Honda engine powering the mixer had a missing cover and was not replaced which exposed employees engaged in cement mixing activities.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  (a) New residential construction located on Lot #503, North side of the structure: On or about 06/08/2017, and at times prior, the employer exposed employees to fall hazards of 22-feet, 5-inches to the lower level while climbing the scaffold frames, in that a ladder or other safe means was not provided to employees engaged in masonry work on the chimney.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  a) New residential construction located on Lot #503, Scaffold plank from roof to tubular welded scaffold system: On or about 06/08/2017, and at times prior, the employer exposed employees to fall hazards while installing bricks from a scaffold plank, in that a fall protection system was not provided while engaged in brick installation from a tubular welded scaffold system.
