29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  On or about June 7, 2017, at the worksite located at 4622 Stadium Boulevard in Jonesboro, Arkansas, north side of building, wooden boards with protruding nails were scattered on the ground.  This condition exposed employees to puncture hazards.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  On or about June 7, 2017, at the worksite located at 4622 Stadium Boulevard in Jonesboro, Arkansas, the grinding wheel on the Makita portable grinder used to smooth sharp edges on metal, was not guarded.  Employees were exposed to the hazard of contact with the rotating wheel.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about June 7, 2017, at the worksite located at 4622 Stadium Boulevard in Jonesboro, Arkansas, the portable ladder used to access and descend from the roof did not extend 3 feet above the landing surface.  This condition exposed employees to falls from elevations.
