29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface (see Figure D-1);    a. On or about June 1, 2017, located at a residential home in Columbus, Ohio, the employer did not ensure that an extension ladder was three feet above the landing surface for employees to safely access to and from the ladder, thereby exposing the employees to a fall hazard of approximately 20 feet.    On June 1, 2017, an employee fell approximately 20 feet while stepping from the eave of a roof and onto a ladder that was not three above the landing surface.

29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    a. On or about June 1, 2017, located at a residential home in Columbus, Ohio, employees cleaned gutters while working from rooftops without any means of fall protection, thereby exposing the employees to a fall hazard of approximately 20 feet.   On June 1, 2017, an employee fell approximately 20 feet off of the roof and onto a ladder without any means of fall protection striking the ground receiving severe injuries.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a.  On or about June 1, 2017, the employer, Dan the Gutterman, did not report the in-patient hospitalization of an employee within twenty-four hours to the Columbus Area Office or the toll free reporting line.
