29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At the jobsite: Employees were tearing down and replacing roof shingles from about 22 feet and 6 inches high from ground level on a 6 in 12 pitch roof without the use of appropriate fall protection. Employees were exposed to fall hazards as observed on or about June 7, 2017. CORRECTED DURING INSPECTION.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated: a) At the jobsite: Employee were working inside the platform of the Genie Z-60/34 boom lift about 22 feet high from ground level without appropriate fall protection, as observed on or about June 7, 2017.
