29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: Location: Facility A TCM LPG forklift, Model Number FCG15N6, S/N 32000961, was not taken out of service, and its exhaust system was emitting very high concentrations of carbon monoxide. This created conditions that presented immediate health hazards to the operator and other employees working in the area.

29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000: Carbon monoxide levels averaged 510 ppm 1 foot away from the truck and 420 ppm 5 feet away. OSHA's Permissible Exposure Limit is 50 ppm. The NIOSH Ceiling Limit is 200 ppm.
