29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a. At the workplace in the meat cutting room, mounted on the North wall, there was a 480 volt disconnect that was used to supply electrical power to the Hobart vertical meat saw, model number 6614, serial number 311477040. The electrical disconnect was blocked by a steel meat cutting table, which did not allow access to the electrical disconnect if an emergency occurred with the equipment and electrical power had to be immediately disconnected.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination shall be made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. a. On or about May 30, 2017, in the cutting room, employees had an occupational exposure to blood while cleaning and sanitizing the Hobart vertical meat saw, model number 6614, serial number 311477040, and working area, after an employee received severe lacerations to the left hand, wrist, and pinky finger. The employer did not offer the hepatitis B vaccination to each covered employee.
