The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a. 29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.   a. On or about June 12, 2017, located at the West Plains Resaw, there were a series of chains and sprockets approximately 36 inches above the floor that were not enclosed with guarding along the bottom for a span of approximately 51 feet, thereby exposing the employee to a caught-in hazard.     b. On or about June 12, 2107, located at the Edmistons Hydraulic Sawmill Equipment powered conveyor system, there were a series of chains and sprockets approximately 36 inches above the floor that were not enclosed with guarding along the bottom for a span of approximately 21 feet, thereby exposing the employees to a caught-in hazard.

29 CFR 1910.265(c)(5)(ii): Handrails. Stairways shall be provided with a standard handrail on at least one side or on any open side. Where stairs are more than four feet wide there shall be a standard handrail at each side, and where more than eight feet wide, a third standard handrail shall be erected in the center of the stairway.  a. On or about June 12, 2017, located at the stairs to the operator's platform at the debarker, there was no handrail for employees to use to safely walk up and down the stairs, thereby exposing the employees to a maximum fall hazard of five feet.

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:  29 CFR 1910.303(b)(1)(iv): Electrical insulation;   a. On or about June 12, 2017, located on the debarker operators platform, there was an extension cord that was used to operate a Stihl electric chain saw, model number MSE 140C and 4-1/2 inch Milwaukee angle grinder, serial number 856E400293476 that had damage to its jacket and insulation revealing the cords conductors, thereby exposing the employee to an electrical hazard of 120 VAC.     b. On or about June 12, 2107, located on the debarker operators platform, there was 4-1/2 inch Milwaukee angle grinder, serial number 856E400293476, that had significant damage its flexible cord revealing the cords conductors, there by exposing the employees to an electrical shock hazard of 120 vac.

On or about June 12, 2017, located at the end of the inspection table, there was an extension cord plugged into a Maxxair drum fan that did not have strain relief provided where the extension cord met the plug.
