29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer is failing ensure that employees are protected from the hazard of amputations. This most recently occurred at 700 Park Street, Beatrice, NE 68310 on May 25, 2017. Employees are exposed to an unguarded ribbon on the FPEC Industrial Mixer. An employee who was performing cleaning activities on the mixer, when they reached their hand up into the mixer to remove a meat product. Their hand made contact with the rotating ribbon, causing an amputation of the right ring and pinky fingers.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.      Landmark Snacks was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1169034, Citation number 1, Item number 1 and was affirmed as a final order on October 21, 2016, with respect to a workplace located at 700 Park Street, Beatrice, NE 68310.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:      The employer is failing to ensure that employees are protected from the hazard of injury and illness identification. This most recently occurred at 700 Park Street, Beatrice, NE 68310 on May 25, 2017. The OSHA Form 300 for the calendar year of 2017 was not completed as required by the regulation to include the specific object or substance that directly injured or made the employee ill.      Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.

29 CFR 1910.147(c)(6)(ii):  The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment  inspection date, employees included, and the person performing the inspection:    The employer is failing ensure that employees are protected from the hazard of potential amputations from not documenting lockout reviews. This most recently occurred at 700 Park Street, Beatrice, NE 68310 on May 25, 2017.   There was no documentation provided by the employer that showed the person performing the periodic inspection and the employees included in the inspection, but not limited to, the FPEC Industrial Mixer.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
