29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing ensure that employees are protected from the hazard of amputations. This most recently occurred at 700 Park Street, Beatrice, NE 68310 on May 25, 2017. Employees are exposed to an unguarded ribbon on the FPEC Industrial Mixer. An employee who was performing cleaning activities on the mixer, when they reached their hand up into the mixer to remove a meat product. Their hand made contact with the rotating ribbon, causing an amputation of the right ring and pinky fingers. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed. Landmark Snacks was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1169034, Citation number 1, Item number 1 and was affirmed as a final order on October 21, 2016, with respect to a workplace located at 700 Park Street, Beatrice, NE 68310.
