On or about June 8, 2017, at 3450 Braselton Hwy, Dacula, Georgia:  An employee performing masonry work was exposed to a fall hazard of approximately 11-feet to 16-feet when he was raised by another employee using a JLG 460 SJ Telescopic Boom Lift while standing on a wooden pallet without using fall protection.

On or about June 8, 2017, 3450 Braselton Hwy, Dacula, Georgia:  An employee performing masonry work was exposed to a fall hazard of approximately 11-feet to 16-feet while working from a roof awning without an effective means of fall protection when he tied a rope to the d-ring on his back and hung his lanyard to a metal stud which did not allow the connector of the lanyard to lock.

On or about June 8, 2017, at 3450 Braselton Hwy, Dacula, Georgia:  Two employees performing masonry work were exposed to a fall hazard of approximately 7-feet while using a stairwell on the jobsite that did not have stair rails installed on the unprotected side of the stairs.
