29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) On or about May 3, 2017; roof area; employees were observed working near the unguarded edges of the roof without being properly secured to protect them from falling an estimated 30-35 feet to ground level while working on the 2-foot parapet wall.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(a)(24): The side rails of through or sidestep fixed ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder:       a) On or about May 3, 2017; stairway; employees were using a ladder to access the roof area in which the side rails did not extend three feet above the roof area, exposing them to a potential fall hazard of ten feet to the floor.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
