29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) WVU Tech Building:  On or about June 6, 2017 and times prior, 2 employees were painting flats, trim and window sills of the exterior part the building while standing on a flat roof up to 15ft. 7in above grade and were not protected from falling to the concrete ground below.

(a) WVU Tech Building:  On or about June 6, 2017 and times prior, an employee working from the platform of a JLG 660SJ boom lift was attached to the designated anchor point with a web lanyard that was not used in conjunction with a shock absorber to reduce fall arrest forces.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a) WVU Tech Building:  On or about June 6, 2017 and times prior, the fall protection systems provided by the Company  were not inspected prior to use and defective equipment, such as a corroded snap hook connected to a 6ft. lanyard with no labels, a fall protection harness with a rusted dorsal d-ring and missing labels was being used by an employee while working at heights of up to 15ft. 7in.
