29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) Orange Hill Place, Lot 35-37: On or about June 13, 2017 and at times prior, the employer exposed employees to trip and fall hazards in that scrap lumber was allowed to accumulate on the lot while employees were framing a residential home.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) Orange Hill Place, Lot 35-37: On or about June 13, 2017 and at times prior, the employer exposed employees to struck-by hazards from flying particles in that they were using a Hitachi pneumatic nail gun without eye protection when installing roof shingles.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a) Orange Hill Place, lot 35-37: On or about June 13, 2017 and at times prior, the employer exposed employees to electrical shock hazards in that employees were using a Rollair portable air compressor without a strain relief provided at the terminal end of the electrical power cord, while installing roof shingles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    a) Orange Hill Place, lot 35-37: On or about June 13, 2017 and at times prior, the employer exposed employees to fall hazards of approximately 20.9 feet from the 10:12 pitch roof to the ground below, in that they were not provided a Personal Fall Arrest System while installing roofing shingles.    b) Orange Hill Place, lot 35-37: On or about June 13, 2017 and at times prior, the employer exposed employees to fall hazards of approximately 11 feet from the balcony to the ground below, in that they were not provided with a personal fall arrest system while framing.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Orange Hill Place, lot 35-37: On or about 13 June, 2017, the employer exposed employees to fall hazards of approximately 10.3 feet from the first floor landing to the ground below, in that the top step of the ladder could not extended 36 inches above the upper landing surface.
