(a) Outback Steakhouse: On or about June 6, 2017 and times prior, two employees were working on securing a section of metal roofing to the edge of the roof while standing and walking inside the 1ft. parapet wall of the newly constructed entrance tower on the roof up to 14ft. 7in. above grade without the use of a fall protection system.

The power cord was damaged exposing the insulated conductors to damage.
