On or before June 13, 2017, employees operated a Yale stand-up forklift without functioning brakes, thereby exposing employees to struck-by hazards. b. On or before June 13, 2017, employees operated a Crown sit-down forklift with a missing seatbelt and inoperable horn, thereby exposing employees to struck-by hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a. On or before June 13, 2017, the employer allowed employees to operate forklifts without conducting a daily examination of the forklift, thereby exposing employees to struck-by or other hazards.

On or before June 13, 2016 where employees used an extension cord to provide power to a portable fan, the strain relief of the plug was pulled away from the outer jacket of the cord, thereby exposing the employees to an electrical contact hazard.
