29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead: Instance A Randall Bearing, Inc. located in Lima, Ohio; On or about June 13, 2017 the employer did not ensure all surfaces were maintained as free as practicable of accumulations of lead in that lead contamination was found at 23.82 mg/100cm2 on the top of the microwave located in the break room. Employees were exposed to hazards associated with lead through ingestion. Instance B Randall Bearing, Inc. located in Lima, Ohio; On or about June 13, 2017 the employer did not ensure all surfaces were maintained as free as practicable of accumulations of lead in that lead contamination was found at 39.83 mg/100cm2 on buttons of a food vending machine located in the break room. Employees were exposed to hazards associated with lead through ingestion. Instance C Randall Bearing, Inc. located in Lima, Ohio; On or about June 13, 2017 the employer did not ensure all surfaces were maintained as free as practicable of accumulations of lead in that lead contamination was found at 4.24 mg/100cm2 on the top of a lunch table located in the break room. Employees were exposed to hazards associated with lead through ingestion. Instance D Randall Bearing, Inc. located in Lima, Ohio; On or about June 13, 2017 the employer did not ensure all surfaces were maintained as free as practicable of accumulations of lead in that lead contamination was found at 18.43 mg/100cm2 on the top of the quality managers desk located in the front office. Employees were exposed to hazards associated with lead through ingestion.

29 CFR 1910.1025(j)(1)(i): The employer did not institute a medical surveillance program for all employees who are or may be exposed at or above the action level for more than 30 days per year: a) Randall Bearing, Inc. located in Lima, Ohio; On or about June 13, 2017 the employer did not implement a medical surveillance program for employees who are or may be exposed to lead at or above the action level, 30 mg/m3, for more than 30 days per year. An employee operating the Okuma Lathe, 2503-02, was exposed to lead at an eight hour time weighted average of 45 mg/m3 which exceeded the action limit of 30 mg/m3 by 150%. Employees were exposed to hazards associated with the inhalation of lead.

Employees operating mills, lathes, and grinders were exposed to hazards associated with lead.

29 CFR 1910.1025(l)(1)(ii): The employer did not train each employee who is subject to exposure to lead at or above the action level, or for whom the possibility of skin or eye irritation exists, in accordance with the requirements of 29 CFR 1910.1025: a) Randall Bearing, Inc. located in Lima, Ohio; On or about June 13, 2017 the employer did not conduct training with each employee who is subject to exposure to lead above the action limit. An employee operating the Okuma LU35-M was exposed to hazards associated with lead exposure and was not informed of the following: a) OSHAs 1910.1025 lead standard and its appendices. b) The specific nature of the operations which could result in exposure to leave above the action level. c) The purpose and a description of the medical surveillance program, and the medical removal protection program including information concerning the adverse health effects associated with excessive exposure to lead (with particular attention to the adverse reproductive effects on both males and females). d) Engineering controls and work practices associated with job assignments. e) Contents of any compliance plan in effect. f) Instructions to employees that chelating agents should not routinely be used to remove lead from their bodies and should not be used at all except under the direction of a licensed physician.
