29 CFR 1926.453(b)(2)(iv): Employee(s) were not always standing firmly on the floor of the basket of extensible and articulating boom platforms:    a) 600 W Touhy Ave, Park Ridge, IL- On or about June 13, 2017, employees were exposed to the hazard of falling up to approximately 40 feet when climbing to and from the basket of a Skyjack SJ66T aerial lift platform while the lift was used as a means of egress to and from balconies.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
