29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:    location: between lower and upper roof  on or about: 6/12/17    a) Employees were not provided with a ladder or any other safe means of access and egress between the lower and upper roof. The employees were climbing up and down the 4 feet change in elevation.    NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface had side rails that extended at least 3 feet above the upper landing surface:     location: lower roof  on or about 6/12/17    a) Employees were using a metal extension ladder to gain access to the 19 feet high roof. The ladder only extended 1 foot above the roof.     NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1910.28(b)(13)(i): 29 CFR 1910.28(b)(13)(i): Work on low-slope roofs. (i) When work was performed less than 6 feet (1.6 m) from the roof edge, the employer did not ensure each employee was protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system:      location: lower roof  on or about: 6/12/17    Employees performing maintenance on the roof, were not provided with any means of fall protection. The employees were exposed to a fall of 19 feet.       NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.      The Mile Square Roofing Co. Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1051132, citation number 1, item number 1 and was affirmed as a final order on 8/11/2015, with respect to a workplace located at 195 Route 17N, Paramus, NJ 07652.
