29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The practical exercise portion of the training for powered industrial trucks was inadequate in that these exercises did not reflect what truck operators would lift, in that operators would have to move a bundle of rebar 13 ft. 5 inch in length along with two helpers walking and guiding the rebar from inside the facility through a narrow doorway, an overhead door with a width of 12 ft.

29 CFR 1910.178(l)(4)(ii): Refresher training in relevant topics shall be provided to the operator when: The refresher training provided to the sit-down forklift operator after being involved in an incident on June 8, 2017 was inadequate in that it did not address operating in narrow aisleways and doorways that were the contributing causes to the incident and an evaluation of the knowledge and skills where an employee was injured when a bundle of rebar fell off the forklift fork and struck an employee on the leg.

On June 8, 2017, near Door #7, two helpers were exposed to a struck-by hazard when the lower part of their body was under the bundle of rebar while holding onto a bundle of rebar hoisted by synthetic slings on the forks of the sit-down forklift located approximately two feet off the ground.

29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On June 8, 2017, near Door #7, two helpers were exposed to a struck-by hazard from a hoisted bundle of rebar that was not properly secured to the forks of the forklift to prevent the bundle from sliding off the forks and striking a helper.
