On or about June 14, 2017, on the east side of the second floor level, an employee operating a Hitachi pneumatic nail gun to perform framing work was not wearing eye protection being exposed to flying debris or nails.

On or about June 14, 2017, on the east side of the second floor level, employees conducting framing operations were not protected from an 10 feet 4 inches fall hazard by the use of fall protection system.
