First Congregational UCC job site in DeKalb, IL - on or about June 14, 2017, boatswain's chairs (single point suspension scaffolds) and their components were not inspected by competent person(s) for visible defects prior to starting restoration tasks on a church's cross and steeple. 29 CFR 1926.451(d)(12): Wire rope clips used on suspension scaffolds were used in a manner that did not comply with the requirements of 1926.451(d)(12)(i)-(vi):    First Congregational UCC job site in DeKalb, IL - on or about June 14, 2017, the employer's inadequate selection and installation of wire rope clips on a single-point adjustable suspension scaffold (boatswains chair) exposed employees to a fall hazard of approximately 120 feet to the ground level:     (a) the employer failed to use the U-bolt clips in accordance with the recommendations of the manufacturer, in violation of 1926.451(d)(12)(ii).     (b) the employer used U-bolt type wire rope clips at the point of suspension for the hoist of a boatswains chair, in violation of 1926.451(d)(12)(v).      (c) the employer unsafely positioned a U-bolt wire rope clip with the saddle on the dead end and the U-bolt on the live end of the wire rope, closest to the upper block of the block & tackle hoist system, in violation of 1926.451(d)(12)(vi).    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable in 29 CFR 1926.454(a)(1) through 29 CFR 1926.454(a)(5):    First Congregational UCC job site in DeKalb, IL - on or about June 14, 2017, employees had not been adequately instructed by a qualified person to recognize and avoid unsafe conditions to control and eliminate fall hazards while performing restoration work on the church's cross and steeple from boatswain chairs (single point suspension scaffolds) up to 120 feet above the ground. The employer failed to adequately provide the following training:    (1) the nature of any electrical hazards, fall hazards and falling object hazards in the work area;  (2) the correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used;  (3) the proper use of the scaffold, and the proper handling of materials on the scaffold;  (4) the maximum intended load and the load-carrying capacities of the scaffolds used; and  (5) any other pertinent requirements of this subpart.
