29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ingition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat. On or about May 22, 2017, at the work site located at US Highway S 287, Vernon, Texas, employees attempting to remove a may-way cover on an above ground storage tank containing oil did not take necessary precautions to prevent ignition of flammable vapors were exposed to fire hazards.
