29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On 6/13/2017, employees from Pablo Pena Roofing, LLC were installing shingles on a roof of a residential building located at 6141 Arbor Green Drive in St. Louis, MO. Two employees were not wearing eye protection while using pneumatic nail guns. Pablo Pena Roofing, LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 102(a)(1), which was contained in OSHA Inspection Number 1162333, Citation 1, Item 1, issued on 9/01/2016, and became Final Order on 10/13/2016 with respect to a workplace located at 7 North Belleville, Freeburg, IL.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 6/13/2017, employees from Pablo Pena Roofing, LLC were installing shingles on a roof of a residential building located at 6141 Arbor Green Drive in St. Louis MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The foreman and four employees were observed on the 10/12 sloped roof approximately seventeen feet above the ground without fall protection, exposing them to a fall to the ground below. Pablo Pena Roofing, LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13), which was contained in OSHA Inspection Number 1164539, Citation 1, Item 1, issued on 7/28/2016, and became Final Order on 12/01/2016 with respect to a workplace located at 5405 Gareth Drive, Weldon Springs, MO.
