29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  At the jobsite located at 1985 S Range Ave Colby KS, Employees engaged in commercial roofing activities were exposed to fall hazards of approx. 19' as measured from the eve to the ground below with a pitch of approx. 4 in 12 in that no fall protection systems were in place.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    At the jobsite located at 1985 S Range Rd Colby KS,  Employees engaged in commercial roofing activities were exposed to fall hazards of approximately 19 feet from the eave of the structure to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence/structure.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.      At the jobsite located at 1985 S Range Rd Colby KS, employees engaged in access and egress from the portable ladder onto the roof of the structure were exposed to fall hazards in that the ladder was located in a parking lot with no protection from vehicular traffic.
