29 CFR 1904.10(a): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log: (a) Facility: On or about June 15, 2017, the employer did not record a Standard Threshold Shift on the 2017 OSHA 300 and 300A Form that was identified for Employee 1 in the right ear on an April 18, 2017 audiometric test. The employee was not retested within 30 days of their audiometric test. (b) Facility: On or about June 15, 2017, the employer did not record a Standard Threshold Shift on the 2017 OSHA 300 and 300A Form that was identified for Employee 2 in the right ear on an April 19, 2017 audiometric test. The employee was not retested within 30 days of their audiometric test. (c) Facility: On or about June 15, 2017, the employer did not record a Standard Threshold Shift on the 2017 OSHA 300 and 300A Form that was identified for Employee 3 in the left ear on an April 18, 2017 audiometric test. The employee was not retested within 30 days of their audiometric test. (d) Facility: On or about June 15, 2017, the employer did not record a Standard Threshold Shift on the 2017 OSHA 300 and 300A Form that was identified for Employee 4 in the right ear on an April 19, 2017 audiometric test. The employee was not retested within 30 days of their audiometric test. (e) Facility: On or about June 15, 2017, the employer did not record Standard Threshold Shifts on the 2017 OSHA 300 and 300A Form that were identified for Employee 5 in the right ear and left ear on an April 20, 2017 audiometric test.
