29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Maintenance Shop, Poston, AZ: Employees were using a Porta Cool unit plugged into a series-connected extension cord and relocatable power tap (RPT). Per UL listing, RPTs are not to be series-connected to other RPTs or connected to extension cords. Employees were exposed to a potential fire hazard.

29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity:  Outside wall of the Maintenance Shop Office: The duplex receptacle supplying power to the Porta Cool unit was wired with reverse polarity. Employees were exposed to an electrical shock hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Maintenance Shop, Poston, AZ: The 100-foot yellow extension cord used to power the Porta Cool unit had a damaged outer jacket which exposed the inner conductors. Employees were exposed to a potential electrical shock hazard.
