29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: (a) Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the seamers and fillers prior to unjamming, cleaning and tearing down the machines. (b) Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the Krones Labeling Machine, prior to cleaning, unjamming and servicing the equipment.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (a) On or about June 8, 2017, a sanitation worker exposed to a pinch point created by a chain and sprocket on the Manu Cameron Model 226, Seamer 2a, while cleaning the seamers, was not required to first: (1) turn off and shut down the machine, including the conveyor, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric, pneumatic and vacuum energy sources, (3) affix lockout or tagout devices to each energy isolating device. (b) On or about September 23, 2017, a seamer operator exposed to a pinch point while unjamming the spider on the Manu Cameron Model 226, Seamer 2a, was not required to first: (1) turn off and shut down the machine, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric, pneumatic and vacuum energy sources, (3) affix lockout or tagout devices to each energy isolating device. (c) Employees who were pulling cans out of the seamers, cleaning inside and outside the seamers, and adjusting the sensors on the seamers, were not required to first: (1) turn off and shut down the machine, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric, pneumatic and vacuum energy sources, (3) affix lockout or tagout devices to each energy isolating device. (d) Employees who were cleaning and replacing parts on the Krones Labeling Machine, were not required to first: (1) turn off and shut down the machine, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric, pneumatic and vacuum energy sources, (3) affix lockout or tagout devices to each energy isolating device. (e) Employees who were working on the fillers, were breaking down the fillers at the end of the shift, greasing the drive shafts on the fillers, plugging air hoses into the drive shafts, and picking up cans and cleaning the floor around the drive shafts, were not required to first: (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric, pneumatic and vacuum energy sources, (3) affix lockout or tagout devices to each energy isolating device.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: (a) On or about June 15, 2017, in the Filling and Seaming Department, Line #1, Seamer 1a, Manu Cameron model 216 at the intake side, the employer did not ensure that the guarding completely enclosed the moving vertical shafts, the conveyor chain and sprocket, and the lifting plate. Employees were exposed to pinch points and nip points. (b) On or about June 15, 2017, in the Filling and Seaming Department, Line #1, Seamer 1a, Manu Cameron Model 216 at the output side, the employer did not ensure that the guarding completely enclosed the rotating spider, moving vertical shafts, the conveyor chain and sprocket and the lifting plate. Employees were exposed to pinch points and nip points. (c) On or about June 15, 2017, in the Filling and Seaming Department, Line 1, Seamer 1b, Manu Cameron model 216 at the intake side, the employer did not ensure that the guarding completely enclosed the moving vertical shafts, the conveyor chain and sprocket and the lifting plate. (j) On or about June 15, 2017, in the Filling and Seaming Department, Line #3, Seamer 3a, Manu Cameron Model 226 at the output side, the employer did not ensure that the guarding completely enclosed the rotating spider, the conveyor chain and sprocket and the lifting plate. Employees were exposed to pinch points and nip points. (k) On or about June 15, 2017, in the Filling and Seaming Department, Line #3, Seamer 3b, Manu Cameron model 226 at the intake side, the employer did not ensure that the guarding completely enclosed the conveyor chain and sprocket and the lifting plate. Employees were exposed to pinch points and nip points. There was also a pinch point outside the machine created by a horizontal moving plate and a bolt. (l) On or about June 15, 2017, in the Filling and Seaming Department, Line #3, Seamer 3b, Manu Cameron Model 226 at the output side, the employer did not ensure that the guarding completely enclosed the rotating spider, the conveyor chain and sprocket and the lifting plate. Employees were exposed to pinch points and nip points. Zwanenberg Food Group (USA) Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1192041, citation number 1, item number 2 and was affirmed as a final order on January 31, 2017, with respect to a workplace located at 3640 Muddy Creek Rd., Cincinnati, Ohio.
