29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. Midwest Contracting Inc. worksite located at 7427 Central Ave., Toledo, Ohio: On or about June 14, 2017, the employer did not ensure an employee was adequately protected from cave-ins while working in an un-protected trench. The trench was approximately 9.00 feet deep and consisted of primarily 'C' soil.
