29 CFR 1910.333(a)(2): Where exposed live parts were not de-energized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved. On or about June 15, 2017, maintenance and supervisory employees were exposed to electrical parts energized to 480 volts where the employees opened electrical panels on mold machines number 2 and number 4 to tap sticking mercury switches where other safety-related work practices were not used.

29 CFR 1910.333(c)(2): Unqualified persons were allowed to work on electric circuit parts or equipment that have not been deenergized under the procedures of paragraph (b) of this section. On or about June 15, 2017, maintenance and supervisory employees were exposed to live electrical parts energized to 480 volts where the employees were permitted to open electrical panels on mold machines number 2 and number 4 to tap sticking mercury switches and had not received electrical training from the employer to act as qualifies persons.

On or about June 15, 2017, maintenance and supervisory employees were exposed to live electrical parts energized to 480 volts where the employees were permitted to open electrical panels on mold machines number 2 and number 4 to tap sticking mercury switches using only safety glasses as personal protective equipment.
