29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   On or about May 5, 2017 in the maintenance shop, where employees worked and maintained forklifts, employees were exposed to crushing hazards, freezing, and electrical hazards of 220 volts on the battery chargers.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tag out procedures.   On or about May 5, 2017 in the maintenance shop, employees service or maintain machinery, not limited to, but includes forklifts. The on-site employer and the contractor did not inform each other of their respective lockout or tag out procedures.  Employees were not informed of DHL Supply Chain's lockout and tag out procedures nor did the employer inform DHL of their lockout and tag out program and procedures, where employees are exposed to crushing, freezing, and electrical hazards in the workplace.
