29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about June 13, 2017: An employee was observed on the roof trusses at the north side of the house installing wood framing elements on a residential structure and was not protected by guardrail system, safety net, or personal fall arrest system, nor were employee provided with an alternative fall protection measures, the employees was exposed to a 20-feet fall hazard.    b) On or about June 13, 2017: An employee was observed on the wall west side top plate installing wood framing elements on a residential structure and was not protected by guardrail system, safety net, or personal fall arrest system, nor were employee provided with an alternative fall protection measures, the employees was exposed to a 20-feet fall hazard.   c) On or about June 13, 2017: An employee was observed on the garage trusses installing wood framing elements on a residential structure and was not protected by guardrail system, safety net, or personal fall arrest system, nor were employee provided with an alternative fall protection measures, the employees was exposed to a 12-feet fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about June 13, 2017: Employees were using an A-frame portable ladder to access the second floor and the ladder did not reach the next level, the employees were exposed to an 8 to 12-feet fall hazard.
