On or about 06/14/2017, at the jobsite at 6798 State highway N., Cottleville, MO, employees working in an excavation that was 5 - 6 feet deep, 18 feet wide, and 35 feet long, engaged in pipe installation operations.  Water was allowed to accumulate in the excavation with no means to eliminate the hazard to include such things as, but limited to, a water pump.
