On or about June 15, 2017 and at times prior thereto, at the worksite located at or near 9317 Millers Pointe Court, Sherwood, Arkansas, an employee was using a portable circular saw to cut lumber for framing without eye protection. This hazard exposed the employee to struck by hazards.

This condition exposed employees to the hazard of falling to the ground below.

This condition exposed employees to fall hazards.
