29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, equipment found to be damaged or defective was being used prior to being repaired. On or about May 15, 2017 at 1439 Industrial Park Blvd., Washington, Georgia where an assured grounding program had been established, the creating employer exposed employees to electrical shock and burns. The discrepancies found include, but are not limited to the following hazards: a. ground pins was broken off in the outlet on the temporary power panel "D". b. receptacle in the temporary panel "D" were not properly installed c. electrical cords were suspended less than seven (7) feet above the finished floor .

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about May 15, 2017 at 1439 Industrial Park Blvd., Washington, Georgia, employees were exposed to a electrical hazard when flexible cords did not connect to the device and to the fittings in ways to prevent tension at joints and terminal screws.
