29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation: a. On or about June 14, 2017, at the workplace, the employer did not properly record column (F) which describes the injury or illness, part of the body affected and object or substance that directly injured employee on the 2017 OSHA 300 Log of Work-Related Injuries and Illnesses, impairing the understandability of the hazards in the workplace. b. On or about June 14, 2017, at the workplace, the employer did not properly record columns (H) and (K) on the 2017 OSHA 300 Log of Work-Related Injuries and Illnesses for injuries that resulted in days away from work (DAFW), impairing the understandability of the hazards in the workplace. c. On or about June 14, 2017, at the workplace, the employer did not properly record columns (I) and (L) on the 2017 OSHA 300 Log of Work-Related Injuries and Illnesses for injuries that resulted in job restrictions and/or job transfer, impairing the understandability of the hazards in the workplace.
