Allegheny Tool and Manufacturing Company Inc.; main shop - On or about 6/14/17, and at times prior thereto, a Roll-in vertical band saw did not have an adjustable guard to safeguard the unused portion of the blade and the blade guide rolls. B.) Allegheny Tool and Manufacturing Company, Inc.; main shop - On or about 6/14/17, and at times prior thereto, the Roll-in band saw did not have guards covering the area at the bottom of the saw housing.

Items being stored included, but were not limited to, a tool box.
