29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.  a)  At the jobsite, on or about 6/6/17:  Workers were using handheld grinders, with cutting wheels installed, and without guards.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:   a)  At the jobsite, on or about 6/6/17:  In the basement, inside the service panel, no cover surrounded the breaker switches, exposing workers to contact live electrical circuits.    b)  At the jobsite, on or about 6/6/17:  In the basement, inside the service panel, no covers were placed over 3 open breaker positions.
