29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about June 12, 2017, an employee using a Makita saw to cut Hardi board siding was not wearing eye protection, exposing himself to being struck by flying debris.

On or about June 12, 2017, an employee working from a JLG lift to install siding was not provided with a body belt and lanyard attached to the basket.  He was being exposed to a 12 feet fall hazard.

On or about June 12, 2017, an employee working on balcony to install siding was not provided with a guardrail system, safety net system or personal fall arrest system.  He was being exposed to a 12 feet fall hazard.
