29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Procedures were not developed to protect employees from an electrical hazard while testing the control box on transformers for Baltimore Power. Proper procedures would include where employees should stand while testing equipment, insurance that the transformer is properly wired, and all doors are closed to minimize exposure to any electrical hazard.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: (a) The employer did not develop electrical safe work practices for employees working on transformers to prevent electric shock or other injuries. On or about June 19, 2017 an employee received an electrical shock while working inside of a transformer.
