29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not deenergized before the employee worked on or was near to them:    a.  On or about June 19, 2017, at a jobsite located at 9000 S. Thomas Rd. Panama City Beach, FL. the employer did not ensure live circuits to which the employees was exposed to were de-energized while replacing a fountain lighting system, exposing employees to the hazards of electrocution.

29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on energized circuits and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools    a.  On or about June 19, 2017, an unqualified employees working on an energized 120 volt electrical power supply while replacing a fountain lighting system, was exposed to an electrocution hazard.
