29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:     (a)  Maintenance employees were not provided with adequate training on the energy control program, in that on or about June 14, 2017, maintenance employees performing servicing and maintenance on the Line 7 Lematic On-Line Slicer, removed the guard on the drive chain and sprocket, while the machine was still running, thereby becoming exposed to nip points created by the chain and sprocket, without first isolating and locking out the disconnects.          (b)  Maintenance employees were not provided with adequate training on the energy control program, in that they opened the cage on the Line 7 Lematic On-Line Slicer to change the bun slicer blade, adjusted the air cylinders, worked on the conveyor belt and unjam bread, without first isolating and locking out the disconnects.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    (1)  turn off and shut down the machine,  (2)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electrical and pneumatic energy sources,  (3)  affix lockout or tagout devices to each energy isolating device.  (4)  prior to starting work on machines that have been lockout out, verify that isolation and deenergization of the machine has been accomplished.     (a)  On or about June 14, 2017, maintenance employees performing servicing and maintenance on the Line 7 Lematic On-Line Slicer, removed the guard on the drive chain and sprocket, while the machine was still running, thereby becoming exposed to nip points created by the chain and sprocket.       (b)   Maintenance employees working on the Line 7 Lematic On-Line Slicer, opened the cage guarding the slicer in order to change the bun slicer blade, adjust air cylinders, work on the conveyor belt and unjam bread, thereby becoming exposed to pinch points, struck by hazards and cuts, without first isolating and locking out the energy isolating sources.

29 CFR 1910.147(c)(4)(ii):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:       (a)  The machine specific energy control procedure for locking out the Line 7 Lematic was incomplete and inaccurate, in that it did not include a specific statement on the intended use of the procedures, specific procedural steps for shutting down, isolating, blocking, securing equipment and for the removal and transfer of lockout devices and the responsibility for them, as well as specific requirements for testing machines to verify the effectiveness of lockout devices.
