29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: a) At the jobsite, on the roof, on or about 6/8/17: Tiebacks for a multi-point adjustable suspension scaffold were woven through a tubular welded frame scaffold where neither the suspension scaffold nor the supported scaffold were designed for this configuration. Workers were exposed to hazards resulting in displacement of the supported scaffold and failure of the tiebacks to perform if placed under loads.
