29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On 20 June 2017, a worker was performing clean-up activities in and around the residence under construction while framing activities were being conducted without the use of a hard hat, exposing the worker to the hazard of being struck to the head by falling tools, materials or debris.

29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable. On 20 June 2017, a framer was using a hand-held Makita Circular Saw to cut pieces of lumber without the use of safety glasses, exposing the framer to the hazard of being struck in the face or eye(s) by flying particles or objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). On 20 June 2017, a worker was performing framing activities while standing on the top plate of the first floor of a residence under construction without fall protection, exposing the framer to a fall hazard of approximately 10 feet 1 inch to the concrete foundation below.
