On or about June 20, 2017, on the residential construction site where employees were exposed to a fall hazard greater than 6 feet when performing roofing work without a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about June 20, 2017, at the north side of the residential construction where employees were exposed to a fall hazard when using an extension ladder to access a roof without extending the ladder side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing.
