29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  (a) Location - 815 North Broadway, Haverhill, MA:  On or about 6/20/17, employees were working on a scaffold that had a platform end more than 18 inches beyond its support.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Location - 815 North Broadway, Haverhill, MA:  On or about 6/20/17, employees were exposed to falls of up to thirteen (13) feet while performing roofing work without fall protection.       The Marcos Chiuchi dba SB Contracting company was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 986108, citation number 1, item number 1 and was affirmed as a final order on November 19, 2014, with respect to a workplace located at 4 Liberty Street, Andover, MA.
