29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:      The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 208 S. 21st  St., Dakota City, Nebraska 68731 on or about June 20, 2017. The employer is failing to ensure that employees who were working on a welded frame scaffold had adequate fall protection provided while working on a scaffold that was over 12 feet above the ground.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:                                                                                                               The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 208 S. 21st  St., Dakota City, Nebraska 68731 on or about June 20, 2017. The employer is failing to ensure that employees who were working from an aerial lift doing scaffold erection activities wore a bodybelt/harness and a lanyard.      Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards as required by paragraph (a)(1) of this section:                                                                                                      The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 208 S. 21st  St., Dakota City, Nebraska 68731 on or about June 20, 2017. The employer is failing to ensure that employees who were working on a welded frame scaffold were trained to the nature of fall hazards as a result of the condition of the scaffold that had multiple deficiencies including, but not limited to; lack of fall protection, mud sills missing under base plates, and not securing uprights with coupling pins or other methods.       Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
