29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level: a. On or about June 20, 2017, in the shipping area, an emergency exit route was obstructed by approximately seven industrial carts that were stored in front of the emergency exit door. The carts created multiple choke points that were as narrow as 8-1/2 inches.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a. On or about June 20, 2017, in the third floor conveyor area, a Patterson industrial fan, model #22, used for air circulation, had a missing grounding pin, thereby exposing employees to electrical shock hazards. b. On or about June 20, 2017, in the second floor desk area, a Patterson industrial fan, model #22, used for air circulation, had a missing grounding pin, thereby exposing employees to electrical shock hazards.
